Filling in the questionnaire

  • To begin the application process, simply complete the questionnaire at the bottom of the page. 
  • Your form will be reviewed, and you’ll receive a response with the next steps within 48 hours.

Initial appointment

  • There will then be an initial appointment to check the sperm count and motility meets the acceptance criteria.
  • The results are reviewed within 2-3 days, and the donor is contacted regarding the outcome.
  • If they meet acceptance criteria, they move on to next stages of screening. 

Screening

  • The next stage involves: filling out a medical history and legal consent forms; blood, urine tests and  a physical examination to exclude any physical abnormality or STD (sexually transmitted disease).
  • There is also a mandatory visit to an independent counsellor to ensure that you have come to the right decision for you.

Beginning donation

  • The application and screening process can take 6-8 weeks.
  • After this has been completed, donation can begin.
  • Donors are given an appointment – some choose to visit on the same day and time each week, whereas others need more flexibility.
  • We’ll work with you to accommodate any work commitments or requirements. It’s preferable that the donor visits once a week if possible, and we collect anything between 20-30 samples.

Where did you hear about us?
General Information: Sperm donation is not a simple process with occasional samples dropped off at the clinic as with blood donation. We require commitment of usually 6-12 months, which involves time-consuming health screening and attendance for at least 20 samples. You are also required by law to be placed on a donor register in order that children born as a result of your donations may gain access to your identity (you are not in any way however financially liable for these children). You would receive compensation for each visit.

6. Do you have access to information on both your paternal and maternal family medical history?
7. Are your immediate family fit and healthy?
8. Any birth defects in your family, eg. cleft palette, spina bifida?
9. Do you/have you ever take drugs intravenously?
10. Have you travelled abroad in the past 6 months?
12. Have you ever been a sperm donor elsewhere?
13. Can you confirm you are normally resident in Nottingham or the surrounding local areas of Derby, Leicester or Lincolnshire? (*If the answer is no please do not apply to donate at our unit! You should search for a clinic that is nearer your home)
